About EMPOWERCARE

In order to create resilient communities and reduce individual frailty and loneliness, EMPOWERCARE developed a holistic community asset approach using researchbased solutions to respond to current gaps in the care of people in the target group (those ages 65+ and those aged 50+ with at least one chronic condition).

EMPOWERCARE had a €7.2 million total budget. The partnership consisted of 13 organisations and numerous observer partners across Belgium, France, the Netherlands and the United Kingdom.

Outputs

An Empowerment Strategy EMPOWERCARE partners implemented, reviewed and assessed best practice and existing evidencebased models in a series of delivery sites across four countries, to produce a Strategy to deliver what matters to individuals and communities in order to manage their own care

Workforce Transformation Based on the interventions implemented to develop the Strategy, a package of resources to support workforce transformation was developed through cross-border sharing of existing and new content, tested and refined, then made freely available to professionals.

A Technology Blueprint Alongside the Strategy work, EMPOWERCARE partners co-created, tested and evaluated initiatives and technology that support enablement at an individual and a community level, focusing on the needs and preferences of the older person when it comes to technology.

A comprehensive evaluation Our evaluation partners assessed all three preceding outputs, making a comprehensive report freely available to underline the value of the work of EMPOWERCARE.
